CAC’s are accountable government property. A report of lost or stolen ID card must be provided before a replacement CAC can be issued.
Initial or Renewal
For initial issue or renewal of any type of military identification (CAC, dependent, or retiree etc.), all DEERS eligible customers must have two unexpired forms of ID to include other specific items for the type of DEERS update being requested.
All documents used for identity purposes in DEERS must be original or certified true copies only.
Of the two forms of identification required, 1 must be an unexpired state or federal issued photo ID.
Second form of ID can be either a valid drivers’ license (not the same as the 1st option), unlaminated SSN card, original or certified true copy of birth certificate, valid passport etc.
*Please contact Customer Support for a complete list of acceptable documents.
IMPORTANT
The sponsor is always required to be present for any DEERS update to a dependent’s record unless a Power of Attorney (POA) has been issued specifically authorizing the ability to update DEERS.
Homeland Security Presidential Directive (HSPD-12) and Federal Identity Processing Standard (FIPS-201) requires TWO forms of identification documents (not expired) for initial issue and replacement of ID cards.
